Saves where a project stands in three short files, then hands the next chat a prompt that picks up exactly there.

What it does

Say handoff, wrap up or save state: Claude rewrites HANDOFF.md with the goal, what is done, open problems, tasks and the next step.
Decisions go into DECISIONS.md with their reasons, append only, so the why is never lost.
A small INDEX.md says which file to read for what, so a new chat starts fast and cheap.
Ends with a continuation prompt to paste into the next chat. Say continue and it reads the files back first.
Keeps HANDOFF.md to 60 lines and checks that no passwords or personal data land in any file.
